It turned out to be\nmalignant melanoma, an aggressive type of cancer (you may have heard\nof it in the skin of people or pets).<\/p>\n\n\n\n<p>\nAn aggressive cancer requires an aggressive\ntreatment\u2026<\/p>\n\n\n\n<p>\nTherefore, more tissue, i.e. part of the jaw\nbone, had to be removed. This is called a partial mandibulectomy.<\/p>\n\n\n\n<p>\nBelow is an X-ray that shows the extent of the\nsurgery (between the red arrows).<\/p>\n\n\n\n<div class=\"wp-block-image\"><figure class=\"aligncenter size-large\"><img loading=\"lazy\" decoding=\"async\" width=\"1081\" height=\"980\" src=\"https:\/\/www.drphilzeltzman.com\/blog\/wp-content\/uploads\/2020\/06\/Oliver-X-ray.jpg\" alt=\"An x-ray of the tumor\" class=\"wp-image-1574\" srcset=\"https:\/\/www.drphilzeltzman.com\/blog\/wp-content\/uploads\/2020\/06\/Oliver-X-ray.jpg 1081w, https:\/\/www.drphilzeltzman.com\/blog\/wp-content\/uploads\/2020\/06\/Oliver-X-ray-768x696.jpg 768w\" sizes=\"auto, (max-width: 1081px) 100vw, 1081px\" \/><\/figure><\/div>\n\n\n\n<p>\nSurgery was uneventful and Oliver recovered\nsmoothly.<\/p>\n\n\n\n<p>\nJust a few hours after this invasive surgery,\nOliver ate!<\/p>\n\n\n\n<p>\nHis nurse wrote: \u201cOliver is awake and walking\naround in his cage. I offered him small amounts of his own\nblenderized dog food and he ate it when I hand fed him. He also ate a\nlittle of the boiled chicken that his owner brought.\u201d<\/p>\n\n\n\n<p>\nWould you have believed that a dog would eat a\nfew hours after a big part of the jaw is removed?<\/p>\n\n\n\n<p>\nHis super dedicated owner then chose to give\nOliver the melanoma vaccine.<\/p>\n\n\n\n<p>\nThis is a very rare situation. We actually have\na vaccine\u2026 against cancer!<\/p>\n\n\n\n<p>\nIt\u2019s definitely not cheap, but it\u2019s very\neffective.<\/p>\n\n\n\n<p>\nAbout 4 months after surgery, Oliver\u2019s owner\nwrote:<\/p>\n\n\n\n<p> \u201cHe had his checkup exam today and chest radiographs. I am extremely happy to report everything looks good.  <\/p>\n\n\n\n<p>\nX-rays are clear and he is running around like\na puppy, playing, eating and is unaffected after his surgery.<\/p>\n\n\n\n<p>\nHe is back to eating his kibble (not softened)\nand chewing on his toys.<\/p>\n\n\n\n<p>\nI cannot thank you enough for your patience,\nprofessional advice on how to proceed, and your amazing skills as a\nsurgeon.<\/p>\n\n\n\n<p>\nForever indebted.\u201d<\/p>\n\n\n\n<p>\nThen I didn\u2019t hear much about Oliver, now 10\nyears old, until early May 2020.<\/p>\n\n\n\n<p>\nHis owner reached out to me because he started\ngagging, coughing and having difficulty breathing while awake, and\nmaking weird sounds while sleeping. He also couldn\u2019t bark anymore\u2026<\/p>\n\n\n\n<p>\nHis family vet did an exam of the mouth, and\nfound a big mass, apparently attached to the roof of the mouth.\nClearly, it was blocking his airway.<\/p>\n\n\n\n<p>\nWe performed emergency surgery the very next\nday.<\/p>\n\n\n\n<p> The mass was the size of a walnut, which is huge for such a small dog. Upon further exam, under anesthesia, the mass was actually not attached to the roof of the mouth, but to the right tonsil.<\/p>\n\n\n\n<p>\nHere is a picture of Oliver just before surgery\n\u2013 you can vaguely see the mass inside the mouth. The only good news: we got it all.<\/p>\n\n\n\n<p>\nThe tonsil really behaves like a lymph node.<\/p>\n\n\n\n<p>\nSince the initial cancer was in the right lower\njaw, the theory is that this second mass could have spread from the\ninitial mass, even though it had been removed completely at the time.\nOf course, it\u2019s hard to prove, which is why it\u2019s only a theory.<\/p>\n\n\n\n<p>\nShortly after surgery, Oliver could breathe\nmuch better.<\/p>\n\n\n\n<p>\nOliver is now recovering and we wish this\ncancer survivor the very best.<\/p>\n\n\n\n<p>\nPhil Zeltzman, DVM, DACVS, CVJ, Fear Free\ncertified<\/p>\n","protected":false},"excerpt":{"rendered":"<p>I met Oliver, a 9 year old Yorkie, in late January of 2019.
