We\u2019ve surgically treated a 3 week old pup and a 23 year old kitty.<\/p>\n\n\n\n<p>Virtually any bone in the body can sustain a fracture: skull, vertebrae, legs, pelvis, tail, ribs\u2026 no bone is spared!<\/p>\n\n\n\n<p>Just in the past 3 weeks, we\u2019ve had:<\/p>\n\n\n\n<p>. 3 dogs who jumped out of the car window<\/p>\n\n\n\n<p>. 1 dog who jumped off the couch<\/p>\n\n\n\n<p>. 1 dog who was attacked by another one.<\/p>\n\n\n\n<p>. 1 dog who was hit by a car.<\/p>\n\n\n\n<p>And that\u2019s just in 3 weeks!<\/p>\n\n\n\n<p>This blog was written to:<\/p>\n\n\n\n<p>. inform you about how fractures happen.<\/p>\n\n\n\n<p>. tell you how they are commonly repaired.<\/p>\n\n\n\n<p>. help you prevent them.<\/p>\n\n\n\n<p><\/p>\n\n\n\n<p class=\"has-text-align-left\"><strong><span style=\"text-decoration: underline\" class=\"underline\">How do bone fractures happen?<\/span><\/strong><\/p>\n\n\n\n<p class=\"has-text-align-left\">Here are the top 10 reasons (in no particular order):<\/p>\n\n\n\n<p class=\"has-text-align-center\">Falls<\/p>\n\n\n\n<p class=\"has-text-align-center\">Cancer<\/p>\n\n\n\n<p class=\"has-text-align-center\">Gunshot<\/p>\n\n\n\n<p class=\"has-text-align-center\">Hit by car<\/p>\n\n\n\n<p class=\"has-text-align-center\">Fights\/Bites<\/p>\n\n\n\n<p class=\"has-text-align-center\">Malformation<\/p>\n\n\n\n<p class=\"has-text-align-center\">Jaw \u2013 Bad teeth<\/p>\n\n\n\n<p class=\"has-text-align-center\">Bone deterioration<\/p>\n\n\n\n<p class=\"has-text-align-center\">Fatigue (repeated trauma)<\/p>\n\n\n\n<p class=\"has-text-align-center\">Various traumas (slipping on ice\u2026)<\/p>\n\n\n\n<figure class=\"wp-block-image size-full\"><img loading=\"lazy\" decoding=\"async\" width=\"1920\" height=\"1022\" src=\"https:\/\/www.drphilzeltzman.com\/blog\/wp-content\/uploads\/2022\/06\/Fracture-couch.jpg\" alt=\"A dog laying on the couch\" class=\"wp-image-1969\" srcset=\"https:\/\/www.drphilzeltzman.com\/blog\/wp-content\/uploads\/2022\/06\/Fracture-couch.jpg 1920w, https:\/\/www.drphilzeltzman.com\/blog\/wp-content\/uploads\/2022\/06\/Fracture-couch-1100x586.jpg 1100w, https:\/\/www.drphilzeltzman.com\/blog\/wp-content\/uploads\/2022\/06\/Fracture-couch-768x409.jpg 768w, https:\/\/www.drphilzeltzman.com\/blog\/wp-content\/uploads\/2022\/06\/Fracture-couch-1536x818.jpg 1536w\" sizes=\"auto, (max-width: 1920px) 100vw, 1920px\" \/><\/figure>\n\n\n\n<p><strong><span style=\"text-decoration: underline\" class=\"underline\">How do we repair bone fractures?<\/span><\/strong><\/p>\n\n\n\n<p>There are multiple types of fractures and multiple ways to repair them. Let\u2019s go over the most common ways.<\/p>\n\n\n\n<p>But wait, not all fractures need surgery.<\/p>\n\n\n\n<p>Some will simply heal with rest, time and pain medication.<\/p>\n\n\n\n<p>Occasionally, healing can be attempted with a splint &#8211; a type of bandage reinforced with a molded piece of plastic (I am not too fond of casts\u2026).<\/p>\n\n\n\n<p>The challenge is to know which fractures can predictably heal with a splint.<\/p>\n\n\n\n<p>For example, fractures of the forearm (radius &amp; ulna) and shin bone (tibia &amp; fibula) in small dogs don\u2019t heal well with a splint, and most need surgery. This is a common misconception.<\/p>\n\n\n\n<p>Many fractures require surgery.<\/p>\n\n\n\n<p>There are 2 main ways to fix a fracture:<\/p>\n\n\n\n<p>. with internal fixation, where no implant can be seen outside the skin.<\/p>\n\n\n\n<p>. with external fixation, meaning that the implants are visible from the outside. This is called an external fixator.<\/p>\n\n\n\n<ul class=\"wp-block-list\"><li>Plates and screws<\/li><\/ul>\n\n\n\n<p>The most common type of repair involves a bone plate and some screws. A plate is a flat stainless steel bar with holes, designed to keep the pieces of bone together while the fracture heals. The plate is attached to the bone with stainless steel screws.<\/p>\n\n\n\n<p>Plates have different shapes, thicknesses, widths and lengths. The number of screw holes varies accordingly.<\/p>\n\n\n\n<p>So a Yorkie with a forearm (radius) fracture may need a small 6-hole plate, and a Lab with a complicated thigh bone (femur) fracture may need a big 12-hole plate.<\/p>\n\n\n\n<p>Therefore, a surgery practice must stock a variety of plates, since we need to be prepared to help a kitten as well as a Great Dane.<\/p>\n\n\n\n<figure class=\"wp-block-image size-full is-resized\"><img loading=\"lazy\" decoding=\"async\" src=\"https:\/\/www.drphilzeltzman.com\/blog\/wp-content\/uploads\/2022\/06\/Fracture-femur-fracture-repaired-with-a-plate-screws-pin-in-a-10-yo-cat-2.jpg\" alt=\"A broken limb repaired with pins and plates\" class=\"wp-image-1973\" width=\"812\" height=\"1162\" srcset=\"https:\/\/www.drphilzeltzman.com\/blog\/wp-content\/uploads\/2022\/06\/Fracture-femur-fracture-repaired-with-a-plate-screws-pin-in-a-10-yo-cat-2-768x1100.jpg 768w, https:\/\/www.drphilzeltzman.com\/blog\/wp-content\/uploads\/2022\/06\/Fracture-femur-fracture-repaired-with-a-plate-screws-pin-in-a-10-yo-cat-2-1072x1536.jpg 1072w\" sizes=\"auto, (max-width: 812px) 100vw, 812px\" \/><figcaption>This 10 year old cat\u2019s femur (thigh bone) fracture was repaired with a plate, 12 screws, several wires &amp; a pin.<\/figcaption><\/figure>\n\n\n\n<ul class=\"wp-block-list\"><li>Wires<\/li><\/ul>\n\n\n\n<p>To help reconstruct a bone that is in several pieces, we sometimes need the help of stainless steel wire (\u201ccerclage wire\u201d).<\/p>\n\n\n\n<p>Some wire is placed around pieces of bone, and the ends are twisted, like a twisty-tie on your bread.<\/p>\n\n\n\n<ul class=\"wp-block-list\"><li>Pins<\/li><\/ul>\n\n\n\n<p>Pins look like knitting needles. They come in various lengths and sizes.<\/p>\n\n\n\n<p>They are used to connect one piece of bone to another. These implants are typically designed to stay in forever. Only if they cause problem would we remove them.<\/p>\n\n\n\n<ul class=\"wp-block-list\"><li>External fixators<\/li><\/ul>\n\n\n\n<p>In some cases, such as bad gunshot wounds, we prefer using \u201cexternal fixation.\u201d<\/p>\n\n\n\n<p>There are different types of external fixators, but here is the concept: several pins are placed through the skin, into the broken pieces of bone.<\/p>\n\n\n\n<p>Then a big metal bar, or some other device, is used to connect all the pins. By keeping the pins together, we indirectly keep the pieces of bone aligned, which helps them heal.<\/p>\n\n\n\n<p>To avoid infection, the holes in the skin must be cleaned regularly, e.g. with hydrogen peroxide.<\/p>\n\n\n\n<p>Because it is external to the skin, the fixator must be removed after the fracture is healed. Not every fracture can be prevented. Accidents do happen.<\/p>\n\n\n\n<p>Some dogs sustain a fracture while playing in the back yard. Some cats get a fracture while playing indoors. As sad as it is, that is nobody\u2019s fault and simply cannot be avoided.<\/p>\n\n\n\n<p>Many could have been prevented. Several easy ways come to mind:<\/p>\n\n\n\n<p>&#8211; Keep your dog on a leash when outdoors to decrease the risk of getting into a fight or being hit by a car.<\/p>\n\n\n\n<p>&#8211; Keep cats indoors.<\/p>\n\n\n\n<p>&#8211; Keep dogs inside a fenced-in yard.<\/p>\n\n\n\n<p>&#8211; Don\u2019t let small dogs jump from your arms or furniture. They can get very common fractures of the forearm or the elbow.<\/p>\n\n\n\n<p>&#8211; Never let your car windows open to allow your dog to enjoy the wind and the smells. Remember, we saw 3 dogs who jumped from a car window in the past 3 weeks. All 3 owners swore that they had done it multiple times and they had never jumped before. Which is true\u2026 until they did.<\/p>\n\n\n\n<p>Fractures can be painful both to the patient and the owner\u2019s finances.
