{"id":2643,"date":"2026-07-06T12:48:42","date_gmt":"2026-07-06T16:48:42","guid":{"rendered":"https:\/\/www.drphilzeltzman.com\/blog\/?p=2643"},"modified":"2026-07-06T12:49:47","modified_gmt":"2026-07-06T16:49:47","slug":"fix-small-problems-before-they-become-big-problems","status":"publish","type":"post","link":"https:\/\/www.drphilzeltzman.com\/blog\/fix-small-problems-before-they-become-big-problems\/","title":{"rendered":"Fix small problems before they become big problems"},"content":{"rendered":"\n<p class=\"wp-block-paragraph\"><strong>A Board-Certified Surgeon&#8217;s Plea to Pet Owners<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">We&#8217;ve had this conversation hundreds of times, and it breaks our heart every single time.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">A pet owner, tears streaming down their face, looking at their beloved dog or cat. There&#8217;s a tumor&nbsp; &#8211; now the size of a grapefruit &#8211; that&#8217;s invaded surrounding tissue, wrapped around blood vessels, or spread to the lungs.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">And almost always, they tell us the same thing:<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">&#8220;It was so small 6 months ago. We were just watching it.&#8221;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">We understand the impulse. We really do.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">No one wants to put their pet through surgery for &#8220;just a little bump.&#8221;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">No one wants to spend money on something that might be nothing.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">And frankly, subconsciously, no one wants to face the possibility that something could be seriously wrong.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">But here&#8217;s the reality: in veterinary medicine, &#8220;wait and see&#8221; is almost never the right answer.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong><u>The Tumor That Started as a Pea<\/u><\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Let us tell you about Max.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Max was a 9-year-old Labrador who was presented with a softball-sized mass on his back leg.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">It was ulcerated, bleeding, and painful.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">His family was devastated.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">&#8220;When did you first notice this?&#8221; we asked.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">&#8220;About a year ago,&#8221; his owner said quietly.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">&#8220;But it was tiny. Like a pea. Our regular vet said we could just monitor it.&#8221;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Here&#8217;s the thing: a year ago, we could have removed that pea-sized tumor quickly and easily.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Max would have gone home the same day.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Instead, we were now looking at a massive surgical procedure &#8211; removing a significant portion of his leg muscle, and possibly amputating the limb entirely, followed by chemotherapy, with no guarantee we could get it all.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The cost would now be much higher.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The recovery would now be longer.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">And Max&#8217;s outcome had gone from excellent to guarded.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">All because they &#8220;just watched it.&#8221;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Now, who in their right mind would pay to remove a mass the size of a pea, or a grape?<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Well, lots of our clients.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Even some pea-size masses had biopsies that came back cancerous.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Size doesn\u2019t matter.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The size of a tumor doesn\u2019t predict whether it\u2019s benign or cancerous.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">But a small mass means a quicker and easier surgery, a shorter anesthesia, less money spent, a quicker recovery, and a better outcome.<\/p>\n\n\n\n<figure class=\"wp-block-image size-large\"><img loading=\"lazy\" decoding=\"async\" width=\"2000\" height=\"1500\" src=\"https:\/\/www.drphilzeltzman.com\/blog\/wp-content\/uploads\/2026\/07\/Belly-mass-2000x1500.jpg\" alt=\"\" class=\"wp-image-2644\" style=\"object-fit:cover\" srcset=\"https:\/\/www.drphilzeltzman.com\/blog\/wp-content\/uploads\/2026\/07\/Belly-mass-2000x1500.jpg 2000w, https:\/\/www.drphilzeltzman.com\/blog\/wp-content\/uploads\/2026\/07\/Belly-mass-1100x825.jpg 1100w, https:\/\/www.drphilzeltzman.com\/blog\/wp-content\/uploads\/2026\/07\/Belly-mass-768x576.jpg 768w, https:\/\/www.drphilzeltzman.com\/blog\/wp-content\/uploads\/2026\/07\/Belly-mass-1536x1152.jpg 1536w, https:\/\/www.drphilzeltzman.com\/blog\/wp-content\/uploads\/2026\/07\/Belly-mass.jpg 2048w\" sizes=\"auto, (max-width: 2000px) 100vw, 2000px\" \/><figcaption class=\"wp-element-caption\">A small mass is easier to remove than a large mass<\/figcaption><\/figure>\n\n\n\n<p class=\"wp-block-paragraph\"><strong><u>Why &#8220;Just Watching&#8221; Rarely Works<\/u><\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">When clients tell us they want to &#8220;watch&#8221; a lump, we just don\u2019t dare asking: &#8220;What are you watching for?&#8221;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The answer would likely be: &#8220;Well, to see if it gets bigger.&#8221;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">And that&#8217;s the problem.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">By the time a tumor is visibly growing, it could be late in the game.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Here&#8217;s what happens when you wait:<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong><u>1. Small tumors become big tumors<\/u><\/strong><br>That pea becomes a grape.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The grape becomes a golf ball.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The golf ball becomes a grapefruit.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">And with every millimeter of growth, the surgery becomes more complicated, more expensive, and riskier.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong><u>2. Even benign tumors can become more complicated<\/u><\/strong><br>Let\u2019s take the example of a tumor on the gum.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">It starts out as small and benign, and then can become \u201clocally aggressive\u201d.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">This means that, although they are benign, they can invade into the jaw bone.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Catching them early means catching them before the invasion.<\/p>\n\n\n\n<figure class=\"wp-block-image size-large\"><img loading=\"lazy\" decoding=\"async\" width=\"2000\" height=\"1500\" src=\"https:\/\/www.drphilzeltzman.com\/blog\/wp-content\/uploads\/2026\/07\/Ear-mass-2000x1500.jpg\" alt=\"\" class=\"wp-image-2645\" style=\"object-fit:cover\" srcset=\"https:\/\/www.drphilzeltzman.com\/blog\/wp-content\/uploads\/2026\/07\/Ear-mass-2000x1500.jpg 2000w, https:\/\/www.drphilzeltzman.com\/blog\/wp-content\/uploads\/2026\/07\/Ear-mass-1100x825.jpg 1100w, https:\/\/www.drphilzeltzman.com\/blog\/wp-content\/uploads\/2026\/07\/Ear-mass-768x576.jpg 768w, https:\/\/www.drphilzeltzman.com\/blog\/wp-content\/uploads\/2026\/07\/Ear-mass-1536x1152.jpg 1536w, https:\/\/www.drphilzeltzman.com\/blog\/wp-content\/uploads\/2026\/07\/Ear-mass-2048x1536.jpg 2048w\" sizes=\"auto, (max-width: 2000px) 100vw, 2000px\" \/><figcaption class=\"wp-element-caption\">A small mass is easier to remove than a large mass<\/figcaption><\/figure>\n\n\n\n<p class=\"wp-block-paragraph\"><strong><u>3. Tumors invade surrounding tissue.<\/u><\/strong><br>Say a small tumor sits nicely in one spot &#8211; easy to remove.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">A large tumor sends \u201cfingers\u201d into muscle, wraps around nerves and blood vessels, and sticks to vital structures.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Now we\u2019re not just removing a lump.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">We\u2019re performing reconstructive surgery.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong><u>4. Tumors spread.<\/u><\/strong><br>Some cancers metastasize, i.e. they spread to the lungs, the liver, or lymph nodes.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Early removal can prevent that spread.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Delayed removal gives cancer a head start.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong><u>5. You lose options.<\/u><\/strong><br>A small tumor gives us options: simple removal, shorter anesthesia time, quicker and simpler recovery.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">A large tumor may mean leg amputation, radiation, chemotherapy, or in some cases, humane euthanasia because surgery is no longer feasible.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong><u>&#8220;But My Vet Said We Could Monitor It&#8221;<\/u><\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">We hear this a lot, and we want to be clear: we\u2019re not blaming your family vet.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Some vets try to be conservative, to avoid putting you through unnecessary procedures and expenses.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">But here&#8217;s the reality: monitoring is only appropriate in very specific circumstances, e.g. usually in an elderly pet with other serious health issues where anesthesia is supposedly \u201ctoo risky,\u201d or in cases where it\u2019s already tested and confirmed it&#8217;s benign.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">In most other cases? Monitoring is a gamble.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">And sadly, the house always wins.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">If your family vet suggests monitoring, ask these questions:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>&#8220;What exactly are we monitoring for?&#8221;<\/li>\n\n\n\n<li>&#8220;At what point would you recommend removal?&#8221;<\/li>\n\n\n\n<li>&#8220;What are the benefits of waiting versus removing it now?&#8221;<\/li>\n\n\n\n<li>&#8220;What are the risks of waiting versus removing it now?&#8221;<\/li>\n\n\n\n<li>&#8220;Should we at least test it to see what we&#8217;re dealing with?&#8221;<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">And if the answer is vague or unsatisfying, get a second opinion from a board-certified surgeon who deals with these masses every single day.<\/p>\n\n\n\n<figure class=\"wp-block-image size-large\"><img loading=\"lazy\" decoding=\"async\" width=\"2000\" height=\"1500\" src=\"https:\/\/www.drphilzeltzman.com\/blog\/wp-content\/uploads\/2026\/07\/Foot-mass-2000x1500.jpg\" alt=\"\" class=\"wp-image-2646\" style=\"object-fit:cover\" srcset=\"https:\/\/www.drphilzeltzman.com\/blog\/wp-content\/uploads\/2026\/07\/Foot-mass-2000x1500.jpg 2000w, https:\/\/www.drphilzeltzman.com\/blog\/wp-content\/uploads\/2026\/07\/Foot-mass-1100x825.jpg 1100w, https:\/\/www.drphilzeltzman.com\/blog\/wp-content\/uploads\/2026\/07\/Foot-mass-768x576.jpg 768w, https:\/\/www.drphilzeltzman.com\/blog\/wp-content\/uploads\/2026\/07\/Foot-mass-1536x1152.jpg 1536w, https:\/\/www.drphilzeltzman.com\/blog\/wp-content\/uploads\/2026\/07\/Foot-mass-2048x1536.jpg 2048w\" sizes=\"auto, (max-width: 2000px) 100vw, 2000px\" \/><figcaption class=\"wp-element-caption\">A small mass is easier to remove than a large mass<\/figcaption><\/figure>\n\n\n\n<p class=\"wp-block-paragraph\"><strong><u>It&#8217;s Not Just Tumors<\/u><\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The &#8220;wait and see&#8221; trap doesn&#8217;t just apply to lumps and bumps.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong><u>Limping<\/u>:<\/strong> &#8220;She&#8217;s a little off on her back leg, but it doesn&#8217;t seem to bother her.&#8221;<br>Dogs and cats hide pain.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">That mild limp could be a torn ACL that&#8217;s getting worse with every walk, leading to arthritis and pain.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Early intervention preserves joint function.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Waiting destroys it.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong><u>Ear infections<\/u>:<\/strong> &#8220;He shakes his head a lot, but we&#8217;ve been treating it at home.&#8221;<br>Ongoing ear infections lead to ruptured eardrums, middle ear infections, and a thickened ear canal that require extensive surgery.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The surgery (a TECA or Total Ear Canal Ablation) is a common one in our hands, but it\u2019s much trickier as time goes by.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong><u>The Money Factor<\/u><\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Waiting doesn&#8217;t save money.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">It multiplies the cost.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">And that&#8217;s just the financial cost.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The emotional cost &#8211; watching your pet suffer through a major surgery, a prolonged recovery, or a poor outcome &#8211; is immeasurable.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong><u>What We Prefer You Do<\/u><\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">If you take nothing else from this blog, please remember this:<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">1. Don&#8217;t ignore lumps and bumps<br>If you find a new mass on your pet &#8211; ANY mass &#8211; get it checked.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Not in 6 months.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Not when you have time.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Now.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">2. Ask for testing<br>There is a quick, inexpensive test, where a needle is stuck into the lump to look at the cells under a microscope.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">It won&#8217;t give us a definitive diagnosis in every case, and sometimes an incorrect negative result, it can usually tell us whether we&#8217;re dealing with something benign or cancerous.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">3. Don&#8217;t wait for symptoms.<br>By the time your pet is limping badly, refusing food, or showing obvious pain, the problem is advanced.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Pets are masters at hiding discomfort.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Subtle changes &#8211; a little stiffness, a slight decrease in energy, a small lump &#8211; are your early warning system.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">4. Trust your gut<br>If something feels off, it probably is.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">You know your pet better than anyone.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">If you notice a change &#8211; even a small one &#8211; don&#8217;t let anyone talk you into ignoring it.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">5. Understand that &#8220;benign&#8221; doesn&#8217;t mean &#8220;ignore it.&#8221;<br>This is probably the most important thing to remember!<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Even benign tumors can grow, bleed, ulcerate, get infected, or interfere with movement and quality of life.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Some benign tumors can become problematic simply due to their size.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">&#8220;Benign&#8221; means it won&#8217;t spread to other organs &#8211; it doesn&#8217;t mean it won&#8217;t cause problems.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong><u>The Conversation We Wish We Never Had to Have<\/u><\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">We never want to talk to another heartbroken pet owner and say, &#8220;We wish we&#8217;d seen your pet 6 months ago.&#8221;<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">We never want to tell someone that their dog&#8217;s leg needs to be amputated because a small tumor was allowed to grow so much bigger.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">We never want to explain that their cat&#8217;s cancer has spread to the lungs and there&#8217;s nothing more we can do.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Yet we have those conversations all the time.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">And almost always, it didn&#8217;t have to be this way\u2026<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong><u>Early is Easy. Late is Hard.<\/u><\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Here&#8217;s the truth: surgery is easier, cheaper, safer, and more successful when problems are small.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">A small tumor removed early has clean edges (clean margins), minimal tissue disruption, and an excellent outcome.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">A large tumor removed late requires extensive surgery, has a higher risk of complications, costs more, and often has a worse outcome.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Small problems stay small &#8211; until they don&#8217;t.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">And once they&#8217;re big, we just can&#8217;t turn the clock back.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong><u>Your Pet is Counting on You<\/u><\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Your dog or cat can&#8217;t advocate for themselves.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">They rely entirely on you to notice changes, to ask questions, to push for answers, and to act when something isn&#8217;t right.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">So please, don&#8217;t wait and watch.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Act and protect.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">That little lump? Get it checked.<br>That slight limp? Get it evaluated.<br>That missed meal? Don\u2019t procrastinate.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Because the best time to fix a problem is before it becomes a crisis.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">And the best surgery?
